Our In-Home Care Team

Our compassionate cares team includes caregivers, who are specially screened, trained, experienced and are licensed, insured and bonded under Love At Home Cares, Inc. will work with you and the family to develop a personal plan of care. This includes specific steps to promote comfort and help improve and maintain your quality of life. You and your loved ones need to know that you are not alone. We are only a phone call away.

Patient

The person requiring any of the levels of care.

Primary Caregiver

This can be an immediate family member, close friend, relative, or Trustee with Power of Attorney/Home Care Directive .

Primary Physician

Your personal physician who continues to be your primary care provider and oversees the treatment plan.

Hospice Physician

The hospice physician works closely with your primary physician and other team members to implement your plan of care.

Case Manager

From Love at Home Cares, who would oversee your plan of care coordinating with your on-site medical provider and Love at Home’s staff.

What happens if the patient’s condition improves?

If a patient’s condition improves to a level where the patient can perform the Activities of Daily Living (ADLs) without assistance or with minimal help from a family member, we ask for at least three (3) days notice to assess the care that may be needed and review safety guidelines with the family before discharge. Love at Home Cares is always available for future home care needs, as appropriate.

In Case of Emergency

If patient is under hospice care, call your hospice care provider. For non-hospice patients call 911.

References

We can provide excellent references upon request.

Love at Home Cares Team members must each clear our extensive screening which includes:

Fingerprint testing with Live Scan Technology

Criminal Background Check with Federal & State

Checked against Sex Offender Registries

Drug tested & Validate evidence of Negative TB Test Results

State Driving Record Checked Annually & Validate Evidence of Auto Insurance (when applicable)

Previous Employment & References are Verified

Skill Orientation Training which includes CPR, First Aid

Bonded and Covered by an Extensive Array of Insurance Policies
